Work Environment

This position is primarily onsite, as you will be delivering care directly within the homes of your clients across New York City. You will have the opportunity to engage with clients in a comfortable, familiar setting, which is essential for effective rehabilitation. Key Responsibilities

• Direct and manage physical therapy treatment plans focused on maximizing physical capabilities, preventing disability, and alleviating pain for each patient.
• Conduct thorough evaluations utilizing diagnostic assessments to determine client functionality and formulate individualized care strategies.
• Supervise Physical Therapy Assistants, ensuring that treatment regimens are effectively executed and aligned with established care plans.
• Assess and recommend modifications to the home environment to facilitate rehabilitation and independence.
• Deliver hands-on therapy, which includes administering exercises, traction, and therapeutic techniques, tailored to each client's condition.
• Collaborate in interdisciplinary team meetings, sharing insights and developments regarding client progress and treatment modifications as necessary.
• Maintain accurate clinical records, including evaluations and patient notes, consistent with OASIS standards, and adhere to documentation timelines for compliance.
• Provide education and support to clients and families, ensuring they understand treatment plans and involve them in their personal care journey.

Patient Population / Client Focus

Our physical therapist will primarily work with diverse populations requiring rehabilitation from various conditions, including post-surgical recovery, chronic pain, neurological impairments, and mobility challenges. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program is required, with a Doctorate preferred.
• Valid current PT licensure in good standing in New York.
• Current CPR certification and a valid driver's license with automobile insurance.
• Exceptional critical thinking, decision-making, and interpersonal communication skills, with a strong dedication to providing high-quality care.
• Ability to demonstrate creativity in treatment planning and a commitment to leading innovative patient care initiatives.

Preferred Experience

• At least one year of relevant experience in a physical therapy role, ideally within acute care or home care settings.
• Proficiency in using regulatory compliance knowledge, particularly federal, state, and governing agency guidelines.
• A history of advocating for patients and families with emphasis on education and engagement in treatment processes.

Compensation and Benefits

This full-time Physical Therapist position offers a competitive annual salary ranging from $90,000 to $100,000, commensurate with experience.
