Key Responsibilities

• Evaluate, plan, and implement individualized treatment programs based on the specific needs and physician recommendations for each patient.
• Administer skilled therapeutic interventions that focus on improving patient mobility, strength, and overall function.
• Engage in direct patient care, utilizing techniques designed to achieve measurable functional improvements in alignment with prescribed care plans.
• Document patient progress and treatment outcomes rigorously to ensure continuity of care and compliance with regulatory standards.
• Educate patients and their families about therapeutic procedures, exercises, and wellness strategies for effective long-term recovery and injury prevention.
• Collaborate with other healthcare providers and specialists to enhance the overall treatment experience and optimize patient pathways to recovery.
• Manage caseloads effectively to meet established productivity and utilization goals.

Patient Population / Client Focus

Your patient demographic will primarily include individuals needing orthopedic rehabilitation, as well as those requiring care in pediatrics, neurology, and sports medicine. As a home care specialist, you will address the unique challenges faced by each patient in their personal environment, tailoring your approach to ensure they receive the most effective and compassionate care possible.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid New Jersey State License to practice Physical Therapy.
• A minimum of two years of clinical experience, preferably with a focus on orthopedics.
• Basic Life Support (BLS) certification from the American Heart Association.
• Exceptional verbal and written communication skills, enabling effective delivery of patient education and collaboration with a healthcare team.
• Proficient in Microsoft Office applications and electronic health record systems.

Preferred Experience

• Experience in home care or outpatient settings providing physical therapy services.
• Familiarity with multidisciplinary teamwork and the integration of evidence-based practice in patient care.
• Demonstrated ability to foster strong relationships with patients, maintaining a supportive and motivating environment.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
