Key Responsibilities\n

• Conduct thorough evaluations of clients' needs, strengths, and challenges through virtual meetings.

• Develop and implement individualized care plans in collaboration with clients and their families, focusing on psychosocial wellbeing.

• Facilitate support groups and provide emotional support to clients and their caregivers dealing with grief and loss.

• Educate clients and families about available resources, services, and community support networks.

• Regularly assess and document client progress in accordance with state regulations and organizational policies.

• Participate in clinical team meetings to discuss patient care strategies and improve service delivery.

Patient Population / Client Focus\n

In this role, you will primarily support clients facing terminal illnesses and their families, aiding them in addressing complex emotional and social challenges. Your work will be central in guiding these individuals through transformative experiences, focusing on quality of life, emotional support, and end-of-life planning within a caring hospice setting.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Current and valid LCSW license in Virginia.

• Strong communication skills, both verbal and written, with the ability to build rapport with a diverse client demographic.

Compensation and Benefits\n
