Key Responsibilities\n

• Conduct ongoing assessments: You will evaluate clients' needs through comprehensive assessments to ensure tailored treatment plans that address their unique mental health challenges.

• Schedule management: Provide supervisors with a detailed schedule of appointments, home visits, and necessary observations to ensure thorough follow-up and care coordination.

• Coordinate home visits: Organize and conduct home visits to gather meaningful insights into clients' living conditions and support systems, thereby enhancing treatment efficacy.

• Implement treatment plans: Oversee the execution of individualized treatment plans, ensuring that interventions are effective and aligned with clients’ goals.

• Participate in team meetings: Engage actively in interdisciplinary meetings to contribute insights on client care and discuss collaborative approaches to treatment challenges.

• Documentation: Complete detailed monthly and quarterly progress notes that document clients' developments and treatment outcomes.

• Training: Organize and facilitate training sessions for team members to ensure compliance and understanding of program plans and documentation procedures.

• Follow-up care: Provide ongoing support through follow-up interviews and additional home visits, ensuring clients feel engaged and encouraged in their recovery journey.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary range of $90,000–$100,000 per year, commensurate with experience and caseload availability.
