Key Responsibilities\n

• Conduct ongoing assessments to identify and address the treatment needs of clients.

• Maintain clear communication with supervisors, healthcare team members, family members, and collateral individuals regarding clients’ progress and treatment plans.

• Provide a structured schedule of appointments and home visits to monitor and support the advancement of clients' treatment goals.
\n
• Coordinate home visits to facilitate in-home observations, chart reviews, and interviews necessary for comprehensive care analysis.
\n
• Ensure effective execution of individualized program plans and monitor their implementation.
\n
• Participate actively in Interdisciplinary Team Meetings to collaborate on client care strategies.
\n
• Document monthly and quarterly progress notes to track client advancement.
\n
• Lead training initiatives for staff to ensure competence in program implementation.
\n
• Provide ongoing support through follow-up interviews and additional home visits as required.
\n
• Act as a liaison with the community and mental health organizations to support developmental goals and outreach.

• Assist in creating and updating training materials that enhance our program offerings.

Your role will involve working with a diverse range of clients, including both adults and youth, across various backgrounds and challenges. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.

• A minimum of two years of counseling experience in a clinical context, demonstrating a commitment to therapeutic excellence.

• A strong understanding of legal and ethical standards surrounding mental health care.

• Exceptional interpersonal and communication skills, with a talent for building rapport with clients from diverse backgrounds.
