Key Responsibilities\n

As a Licensed Professional Clinical Counselor (LPCC), you will be expected to:

• Conduct comprehensive, ongoing assessments of mental health needs for each patient, utilizing evidence-based practices.

• Facilitate communication among supervisors, direct care team members, families, and guardians to ensure cohesive and progressive patient care.

• Develop and share a tentative schedule of appointments and home visits, ensuring accountability for both personal and team objectives.

• Coordinate in-home visits to conduct observations and interviews that adhere to best practices in mental health service delivery.

• Evaluate and ensure the effective execution of individualized program plans designed to meet patient needs.

• Participate actively in Interdisciplinary Team Meetings and other relevant discussions to collaborate on best practices and patient care strategies.

• Document monthly and quarterly progress into patient files, maintaining accurate and readily available records for review.

• Organize training sessions to ensure team members are well-prepared and equipped to implement program plans and meet documentation standards.

• Provide continuous support through follow-ups, home visits, and observations to monitor patient progress.

• Serve as a liaison with the wider community and mental health professionals to foster relationships and ensure the alignment of treatment goals.

• Assist in crafting procedure manuals and training materials aimed at improving service delivery and program efficiency.

Compensation and Benefits\n
