In this full-time remote position, you will serve a pivotal role within our Hospice Center, focusing on improving the quality of life for patients and their families during some of their most challenging times. Key Responsibilities\n

Your primary responsibilities will include, but are not limited to:

• Conducting comprehensive initial intake assessments to understand the unique strengths and needs of each client.

• Independently creating and executing effective treatment plans that outline clear, measurable goals and appropriate intervention strategies tailored to individual client needs.

• Providing therapy sessions aimed at fostering personal and interpersonal growth, while actively assisting clients in navigating their emotional challenges.

• Evaluating the effectiveness of interventions regularly to ensure that clients receive optimal support throughout their treatment journey.

• Maintaining minimal documentation, including an initial intake note, an individualized treatment plan, and regular progress notes following therapy sessions.

• Collaborating with interdisciplinary teams to create a seamless support network for clients and their families.

Patient Population / Client Focus\n

In this role, you will work primarily with hospice patients facing end-of-life issues, providing emotional support and counseling to help them during this sensitive phase of life. You will also engage with their families to equip them with coping strategies and resources, ensuring a holistic support system that respects their shared experiences and individual needs.

Qualifications\n

To be considered for this position, candidates must possess:

• A Master's Degree in Social Work or a closely related field.

• An active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two (2) years of experience in mental health or a related field.
\n
• Strong written and verbal communication skills, essential for effective therapeutic relationships and documentation.

• The ability to work both independently and within a collaborative environment, demonstrating strong organizational and time management skills.

Preferred Experience\n
