Work Environment

This position will be based primarily in a home care setting, allowing you to engage directly with patients in the comfort of their own homes. Key Responsibilities

• Conduct comprehensive evaluations and assessments to determine patients' physical therapy needs, leading to tailored treatment plans that promote recovery and enhance physical function.
• Implement therapeutic interventions such as exercises, manual therapy techniques, and functional training to support patients in achieving their rehabilitation goals.
• Educate patients and their families on physical health management strategies, providing guidance on exercises and wellness techniques to facilitate ongoing care.
• Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and social workers, to develop and maintain coordinated patient care that addresses all aspects of health.
• Document all patient interactions and treatment progress accurately in compliance with legal and ethical standards to ensure continuity of care.
• Participate in clinical meetings and training sessions to enhance skills and share knowledge, as well as to contribute to the development of best practices in physical therapy.

Patient Population / Client Focus

The primary focus of this role is on individuals receiving home care services who require assistance with physical rehabilitation. Qualifications

• Active and valid state license as a Physical Therapist.

Compensation and Benefits

• The salary range for this position is USD 115,000 – 130,000 annually, commensurate with experience and qualifications.
