You will work collaboratively with a diverse group of clinicians, including nurses, physicians, and other social workers, to ensure cohesive treatment plans tailored to patients’ unique needs. Key Responsibilities\n

• Conduct thorough clinical evaluations of patients upon admission, assessing their psychological and social needs while accurately identifying any risk of suicidality or danger to self or others.

• Initiate discharge planning on the first day of admission, collaborating with healthcare providers to create effective transition plans that include coping strategies, necessary resources, and follow-up appointments with outpatient services.
\n
• Facilitate therapy sessions and psychoeducational groups, using evidence-based interventions to support patients in processing their emotions and experiences related to their illness.
\n
• Manage crises effectively, providing immediate support during challenging situations both in individual sessions and within the milieu setting.
\n
• Actively coordinate with referral sources and other outpatient providers to establish continuity of care, ensuring all stakeholders are informed of the patient’s needs and discharge plans.
\n
• Lead and participate in program outreach initiatives, promoting holistic approaches to care and enhancing community awareness around hospice services.
\n
• Maintain comprehensive clinical documentation including initial assessments, SMART treatment plans, and clinical case notes, ensuring timely submission and accuracy as required by organizational protocols.

• Support the milieu staff through training, mentorship, and offering actionable guidance during interactions with patients and families.
\n
• Communicate regularly with the Utilization Review Department to assure compliance with payment authorization processes.

Patient Population / Client Focus\n

This position primarily focuses on supporting patients enrolled in hospice care, many of whom are experiencing terminal illnesses and require compassionate emotional and psychological support. You will address a diverse range of needs, helping patients and their families navigate complex emotions, grief, and preparation for end-of-life transitions.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related field is required.

• Must possess an active and unrestricted license as a Licensed Clinical Social Worker (LCSW).
\n
• Demonstrated proficiency in managing boundaries and maintaining professional integrity in patient interactions.
\n
• Ability to work effectively with individuals from diverse backgrounds and cultures.
\n
• Strong organizational skills with the ability to complete detailed documentation in a timely manner.
\n
• Excellent verbal and written communication skills, including experience with using Office 365 and Electronic Health Record systems.

Preferred Experience\n

• Prior experience in clinical settings related to hospice or palliative care is highly desirable.

• Familiarity with crisis intervention strategies and techniques, especially in end-of-life care contexts.
