Key Responsibilities\n

• Act as a vital liaison between families and outpatient healthcare providers, ensuring all parties are informed and supported throughout the care process.

• Engage with insurance companies to facilitate pre-certification and manage utilization effectively, navigating the complexities of healthcare funding and resource availability.

• Take part in comprehensive discharge and aftercare planning, ensuring that services provided address the ongoing needs of clients post-discharge.

• Provide therapeutic meal support to clients, demonstrating and fostering a healthy relationship with food as part of their holistic care.

• Deliver safe, empathetic, and affirming care that is sensitive to the diverse identities and needs of clients, helping them navigate their unique challenges.

Patient Population / Client Focus\n

This role primarily focuses on older adults receiving hospice care, recognizing the unique physical and emotional challenges they face during this significant life stage. Compensation and Benefits\n
