Key Responsibilities\n

• Advocate for clients, helping them navigate community services that enhance their access to healthcare, social activities, and supportive resources.

• Regularly evaluate clients' living conditions through virtual check-ins to monitor their well-being and safety, adjusting care plans as needed.

• Facilitate discussions among family members to promote understanding and cooperation in caring for elderly relatives.

• Educate clients and families on available resources like support groups and local services that can improve their quality of life.

• Maintain accurate, detailed documentation of client interactions, assessments, and care plans using electronic health record (EHR) systems.

Qualifications\n
