Key Responsibilities\n

• Collaborate on the development and implementation of Individualized Family Service Plans (IFSP) tailored to meet the unique needs of families.

• Actively monitor and report on patient outcomes, ensuring continuous improvement of service delivery.

• Provide ongoing support and education to families, empowering them with strategies to enhance their loved one’s quality of life.

• Utilize evidence-based practices while adhering to Natural Learning Environment Practices to facilitate effective interventions.

Patient Population / Client Focus\n

In this role, you will primarily focus on providing care and support to families dealing with the emotional and psychological challenges associated with hospice care. Your sensitivity and understanding will be pivotal in helping these families navigate complex emotional waters.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) issued by the Department of Public Health.
