Work Environment\n

This full-time position allows for a flexible remote work environment that supports work-life balance. While primarily virtual, you will engage with patients both online and in-person when necessary, accommodating client schedules, which may include evenings and weekends. Key Responsibilities\n

• Utilize your expertise in social work to independently assess and formulate psychosocial diagnoses for patients, tailoring interventions based on individual needs.

• Collaborate with patients to create and implement holistic, patient-centered treatment plans that may include therapeutic modalities such as motivational interviewing and problem-solving therapy.

• Conduct regular screenings and evaluations to assess treatment efficacy and patient progress, maintaining ongoing communication with patients and their families.

• Partner effectively with multidisciplinary teams, including clinicians and psychologists, to ensure a comprehensive approach to care that reflects the best practices in mental health intervention.

• Get involved in psychotropic medication management by monitoring adherence as directed by prescribing medical providers, assessing side effects, and measuring treatment outcomes.

• Document patient interactions, treatment updates, and case notes accurately, ensuring compliance with all relevant regulatory and ethical standards.

Your focus will be on supporting individuals in hospice care who are navigating complex emotional and psychological challenges at a critical time in their lives. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid license to practice independently as a Licensed Clinical Social Worker (LCSW) in the state of Arizona.

• A minimum of 1 year of direct experience providing therapeutic support as an independently licensed therapist.

Compensation and Benefits\n

Aloha Healthcare Center offers a competitive salary ranging from $80,000 to $100,000 annually, commensurate with experience, plus additional performance-based bonuses. Benefits include:
