Key Responsibilities\n

• Conduct comprehensive assessments of patients’ psychological and emotional needs, ensuring accurate diagnoses and treatment planning.

• Maintain detailed and confidential patient records, including treatment notes and progress reports, adhering to HIPAA and company policies.

Patient Population / Client Focus\n

Your primary focus will be working with patients experiencing trauma, anxiety, and depression, specifically within the hospice care environment. You will engage with patients and their families, understanding the unique challenges they face during end-of-life transitions, and providing emotional support that empowers them to navigate this difficult journey.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• An active LCSW (Licensed Clinical Social Worker) license in the state of Connecticut.

• Strong communication skills, along with the ability to develop trusting therapeutic relationships with patients and families.

• Proven problem-solving skills and the capacity to handle sensitive situations with compassion and professionalism.
