Work Environment\n

This position is primarily remote, allowing social workers to conduct assessments, consult with families, and collaborate with interdisciplinary teams from the comfort of their homes. However, occasional visits to the hospice center may be necessary for team meetings and essential client interactions. Our virtual tools and platforms support seamless communication, enabling effective teamwork and patient service continuity. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify family needs and challenges related to hospice care.
\n
• Develop and implement Individualized Family Service Plans (IFSPs) that detail specific interventions and support services individualized for each family.
\n
• Actively monitor and evaluate the outcomes of interventions, adjusting as necessary based on client needs and progress.
\n
• Provide emotional support and counseling to families, ensuring they have the tools and resources needed to navigate the end-of-life care process.
\n
• Assist families in understanding their rights and the entirety of available health services, providing information on additional community resources as needed.
\n
• Foster a supportive and collaborative atmosphere, developing strong relationships with children, families, and team members to promote effective learning and growth.
\n
• Utilize evidence-based practices for therapy and intervention strategies, continually aligning with the principles of Natural Learning Environment Practices.
\n
• Ensure compliance with all local, state, and federal regulations that govern hospice services and social work practice.

Patient Population / Client Focus\n

As an LCSW at Mindful Walkway Counseling & Wellness Center, you will mainly work with families facing critical and terminal illnesses, guiding parents and guardians in making informed decisions about their children's care. Our focus on the youngest among us allows you to play an essential role in fostering developmental progress for infants and toddlers with disabilities or delays, while also addressing the emotional complexity families experience during this time.

Qualifications\n

• Master’s Degree in Social Work or a Doctorate in Social Work.

• Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health.
\n
• A valid driver’s license is required for any necessary home visits or access to community resources.
\n
• Demonstrated adaptability and creativity in modifying therapeutic approaches to suit various learning styles and family dynamics.

• Strong organizational and time management skills to effectively manage caseloads and documentation.
\n
• Exceptional communication skills, both verbal and written, to build rapport with families and collaborate with team members.

Preferred Experience\n

• A minimum of one year of experience working with children in early intervention settings.

• Experience in hospice or palliative care is a plus.

Compensation and Benefits\n
