You will interact regularly with healthcare professionals, including primary care providers and psychiatric consultants, to create a seamless integration of care for our patients. Key Responsibilities\n

• Build rapport swiftly with patients and their families, employing active listening and effective communication that respects their preferred communication styles.

• Serve as an integral member of a multidisciplinary team, collaborating with various healthcare providers to develop comprehensive care plans.
\n
• Conduct brief, evidence-based assessments to identify mental health and substance use issues, arranging detailed evaluations as appropriate.
\n
• Manage a patient caseload, providing follow-up interventions and documenting encounters within 48 hours in the electronic health record (EHR) system.
\n
• Develop and implement integrated care plans that ensure patients have access to necessary services and support, while maintaining strict confidentiality of patient information.
\n
• Engage actively in staff meetings, case conferences, and quality improvement initiatives to enhance service delivery and clinical outcomes.
\n
• Facilitate referrals to community resources to support patients' diverse needs, including vocational, cultural, and spiritual services.
\n
• Maintain up-to-date knowledge of behavioral health practices and stay informed about referrals for patients prescribed psychiatric medications.
\n
• Respond to calls from patients, family members, and other stakeholders efficiently, addressing concerns and appointment requests in a timely manner.

Patient Population / Client Focus\n

The focus of this role is on individuals in detoxification and recovery, as well as those experiencing substance use disorders and related mental health issues. You will work with a diverse population, including underserved communities and individuals experiencing homelessness, ensuring culturally competent care that respects each patient's unique background and circumstances.

Qualifications\n

• Master’s degree in a related field of Behavioral Health.

• Current LCSW license is required.
\n
• CPR or BLS certification.
\n
• A minimum of three years (preferably five) of professional experience in the behavioral health field.

• Strong understanding of trauma-informed care and social justice practices.

• Proficient in using electronic health record systems and other digital tools for documentation and communication.
\n
• Bilingual in English and Spanish is preferred to assist diverse patient populations.
