Key Responsibilities\n

• Conduct comprehensive assessments and evaluations to develop personalized treatment plans for each patient.

• Facilitate open communication with supervisors, healthcare team members, family members, and other stakeholders regarding patient progress and treatment adjustments.

• Maintain a well-organized schedule that includes appointments, home visits, and therapeutic observations to monitor client development.

• Conduct home visits, utilizing observation and interaction to gather valuable insights into clients' environments and behaviors.

• Ensure effective implementation of therapeutic program plans and assess their efficacy regularly.

• Participate actively in interdisciplinary team meetings to collaborate on holistic approaches to patient care.

• Document and complete detailed progress notes monthly and quarterly to track individual patient outcomes and engagements.

• Organize and lead training sessions to enhance the skills of team members on implementing effective treatment methodologies and documentation processes.

• Provide ongoing support through follow-up interviews and home visits, adapting treatment plans as necessary to meet evolving client needs.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
