Work Environment

As a Physical Therapist working onsite in a home care setting, you'll be responsible for treating patients within the comfort of their own homes. Key Responsibilities

• Provide ongoing consultation and support to patients and their families, training them on exercises and care routines to encourage self-management and long-term recovery.
• Engage effectively in multidisciplinary team meetings to discuss patient progress and collaboratively adjust treatment strategies as necessary.
• Maintain meticulous records of patient interactions and outcomes to monitor progress and fulfill documentation requirements.
• Aim to meet key performance indicators related to patient utilization and satisfaction, contributing to the overall objectives of the organization.

Patient Population / Client Focus

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited physical therapy program.

• Exceptional communication skills, both verbal and written, to effectively collaborate with patients, families, and healthcare team members.
