Key Responsibilities\n

• Work closely with physicians and other health care professionals to effectively communicate patient progress, needs, and concerns, ensuring comprehensive client care.

• Coordinate with family members and support systems regarding treatment planning and discharge processes, promoting family involvement in the recovery journey.

• Maintain accurate, timely, and confidential documentation, including clinical notes and progress assessments, in accordance with organizational policies.

• Collaborate with the healthcare team to develop personalized treatment and discharge plans based on detailed psychosocial assessments.

• Facilitate care coordination to assist clients with accessing various essential health services in the community and ensure smooth transitions of care.

Qualifications\n
