LPN

The Licensed Practical Nurse (LPN) provides nursing care by administering medications, monitoring resident conditions, reporting changes to the charge nurse and physicians, and assisting Certified Nursing Assistants (CNAs) in a long-term care setting. This role requires strong attention to detail, communication with families and healthcare providers, and adherence to care standards. A valid Wisconsin LPN license and CPR/BLS certification are required, with preference given to candidates with at least one year of related experience. Responsibilities:

• Administer medications to residents following the 5 Rights protocol.
• Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
• Notify the charge nurse of any changes in resident condition.
• Assist CNAs with call light response and other care duties.
• Update physicians and family members regarding resident condition changes as necessary.
• Perform additional duties as assigned.

• Current Wisconsin Licensed Practical Nurse (LPN) license.
• Current CPR/BLS certification.
• Exceptional accuracy and attention to detail.
• At least 1 year of related nursing experience preferred.

• Experience in long-term care or nursing home environment.
• Strong communication and interpersonal skills.
• Ability to collaborate effectively with interdisciplinary teams.

• Must comply with all regulatory and facility policies regarding resident care and safety.
• Responsible for mandatory reporting of abuse or neglect.
• Work environment includes long-term care residents with varying levels of acuity.

• Full-time position.
• PM shift from 2:00 PM to 10:00 PM.
• Work schedule includes every other weekend.
