LPN

The Licensed Vocational Nurse (LPN) will provide skilled nursing care in a Long Term Care (LTC) setting, performing patient assessments, treatments, medication administration, and collaborating with medical providers. Responsibilities include managing ADL assistance, wound care, gastrostomy and urinary catheter care, and other nursing duties to support resident health and comfort. The role requires adherence to facility protocols and effective communication with the interdisciplinary team. Responsibilities:

• Perform patient assessments and administer treatments and medications as prescribed.
• Notify physicians or nurse practitioners of pertinent patient information.
• Assist with admitting and discharging residents.
• Provide assistance with activities of daily living (ADLs).
• Manage gastrostomy tube care, urinary catheter care, nebulized treatments, wound vac care, and CPAP care.
• Document nursing care and patient status accurately and timely.
• Collaborate with interdisciplinary team members to ensure quality resident care.
• Comply with facility policies, including infection control and safety protocols.

• Current Licensed Vocational Nurse (LPN) or Licensed Practical Nurse (LPN) licensure in the state of practice.
• Minimum of 2 years of nursing experience, preferably in Long Term Care or similar settings.
• Skills in patient assessment, medication administration, and treatment procedures.
• Ability to communicate effectively with medical staff and residents.
• Proof of flu vaccination or approved medical/religious exemption prior to employment.

• Experience with wound care, gastrostomy, catheter care, and respiratory treatments.
• Familiarity with LTC facility operations and resident care protocols.
• Basic Life Support (BLS) certification.

• Must comply with facility vaccination policies including flu vaccination requirements.
• Work performed in a Long Term Care environment with elderly and vulnerable populations.
• May be required to work varied shifts based on census and facility needs.

• Shift assignments may change to meet resident care demands.
