Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing pertinent medical histories, performing assessments, and documenting findings to establish individualized treatment plans.
• Communicate evaluation results and collaboratively set objectives for patient care with both patients and their families, ensuring that they are engaged in their recovery journey.
• Effectively implement treatment modalities tailored to the unique needs of each patient, applying evidence-based protocols and adapting approaches as health conditions evolve.
• Collaborate with interdisciplinary teams, consulting with other healthcare professionals as necessary to monitor progress and make informed modifications to care plans.
• Oversee discharge planning and provide comprehensive follow-up care, ensuring smooth transitions for patients leaving therapy and maintaining accurate documentation throughout the treatment process.

Qualifications

Compensation and Benefits
