Key Responsibilities\n

• Conduct thorough assessments to identify the specific mental health needs of patients during the initial evaluation and throughout the therapeutic process.

• Regularly communicate and coordinate with supervisors, direct care staff, and family members to monitor individual patient progress, ensuring transparent engagement throughout treatment.

• Develop and provide a tentative schedule of appointments, home visits, and necessary observations to supervisors, supporting structured and effective therapeutic interventions.

• Conduct home visits that align with therapy goals, involving chart reviews and analysis to evaluate effectiveness and adapt care plans as needed.

• Ensure the effective implementation of treatment plans and monitor all aspects of patient care in accordance with legal and ethical standards.

• Engage actively in interdisciplinary team meetings, sharing insights and contributing to collaborative care efforts.

• Document monthly and quarterly progress notes accurately, maintaining up-to-date records that are easily accessible to authorized parties.

• Organize and facilitate training for staff on the effective implementation of the program plan and documentation practices.

• Provide continuous follow-up support through interviews and home visits to address evolving client needs.

• Act as a liaison between the agency and the wider mental health community, promoting awareness and development of treatment programs.

• Assist in developing training materials and procedures aimed at improving interventions tailored to individual clients.

Qualifications\n
