Key Responsibilities\n

• Conduct thorough bio-psychosocial evaluations to identify barriers to treatment and enhance patients' overall health status.

• Develop individualized treatment plans and conduct both individual and group therapy sessions using evidence-based methodologies.

• Implement patient-centered interventions that address the psychosocial needs of patients and their families, ensuring comprehensive care planning.

• Document all social work activities in patients' medical records meticulously, adhering to departmental standards and best practices.

• Collaborate with medical and support staff to provide holistic care that increases treatment compliance and coordination, fostering a high level of patient satisfaction.

• Educate patients and their families on medical and social work recommendations to promote understanding and acceptance of treatment goals.

• Remain updated on community resources to effectively advocate for patients, particularly emphasizing self-determination and empowerment.

Patient Population / Client Focus\n

The clinician in this role will primarily engage with a diverse array of clients dealing with a spectrum of psychosocial issues, including those facing mental health challenges exacerbated by environmental and personal factors. Our focus is on providing care to vulnerable populations within the Buffalo area, with a commitment to understanding the cultural, developmental, and individual needs of each patient.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution, along with a current LCSW license from the State Board of Social Work Licensure.

• Demonstrated understanding of human development and aging processes, as well as cultural competencies.

• Prior clinical experience of between six months to one year, ideally in a healthcare or mental health environment.

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 85,000 to 100,000 annually, commensurate with experience and qualifications.
