Key Responsibilities\n

• Collaborate with interdisciplinary healthcare teams to create comprehensive treatment plans, including discharge strategies, based on thorough psychosocial assessments and established social work practices.

• Build and maintain relationships with community resource leaders, representing Redesign Counseling Hub at meetings and committees that address the social and health needs of our clients.

• Provide individualized counseling and crisis intervention, conducting thorough assessments and implementing treatment modalities to address emotional and behavioral challenges effectively.

• Maintain open lines of communication with physicians and other healthcare providers regarding patient progress, concerns, and potential issues, ensuring a holistic approach to client care.

• Document clinical sessions and progress notes meticulously, adhering to departmental policies and maintaining confidentiality in all records.

Patient Population / Client Focus\n

In this role, you will work primarily with a diverse range of clients, including children, adolescents, adults, and families experiencing a variety of mental health issues. You will provide care for individuals dealing with anxiety, depression, trauma, relationship difficulties, and significant life transitions. Qualifications\n

Compensation and Benefits\n

The successful candidate can expect a competitive annual salary range of $100,000 – $105,000, complemented by a robust benefits package.
