Key Responsibilities\n

• Engage in proactive counseling sessions with clients to mitigate crises and reduce reliance on emergency services.

• Maintain thorough documentation of client interactions, crisis alerts, and care plans to facilitate coordination with county crisis services.

• Manage transitions between inpatient hospital services and outpatient care, collaborating with hospital staff to ensure smooth discharges and continuity of care.
\n
• Customize and implement treatment plans based on client assessments, focusing on individualized goals and measurable outcomes.
\n
• Conduct regular follow-up sessions to monitor progress, adjust treatment strategies, and provide ongoing support to clients.

• Prepare comprehensive assessments and evaluations to inform clinical decision-making and resource allocation.

Patient Population / Client Focus\n

The primary clients you will work with present a wide range of psychiatric, social, and emotional challenges, often experiencing significant life stressors. This includes individuals facing anxiety, depression, relationship issues, trauma, and other mental health concerns. You will work with a diverse population, emphasizing culturally informed practices that honor each client's unique background and experiences.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling is required.

• Active licensure as a Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).

• Ability to establish rapport quickly with clients from various backgrounds while demonstrating empathy and respect.

• Strong communication skills, particularly in conveying sensitive information in a culturally competent manner.

• Familiarity with telehealth systems and electronic health record management is an advantage, as well as experience in remote counseling methods.

Preferred Experience\n

• Prior experience working in a telehealth capacity or with remote patient care is highly desirable.

Compensation and Benefits\n
