Key Responsibilities\n

As a Telehealth Professional Counselor, you will:

• Work proactively with clients to avoid crisis services, emphasizing preventive measures and community support.

• Complete and maintain detailed crisis alerts to ensure effective coordination with county crisis services for each participant.
\n
• Oversee and facilitate transitions in hospital or inpatient services, which includes preparing necessary documentation and coordinating with healthcare staff to support safe discharges.

• Engage in ongoing assessments and evaluations to identify clients’ needs and contribute to the personalized care planning process.
\n
• Document therapeutic interventions and maintain accurate patient records in accordance with regulatory standards and clinic policies.

Patient Population / Client Focus\n

The ideal candidate will engage with a diverse client population facing a variety of psychiatric, social, and emotional challenges. You will handle cases ranging from anxiety and depression to more acute mental health concerns, using telehealth modalities to build rapport and trust. Qualifications\n

To excel in this role, candidates must possess:
