Key Responsibilities\n

In this role, you will be responsible for:

• Conducting comprehensive assessments and maintaining crisis alerts to coordinate care with county crisis services for program participants.

• Working closely with hospital staff to facilitate smooth transitions between inpatient services and outpatient care, ensuring appropriate follow-up and support during discharges.

• Employing effective crisis de-escalation strategies to manage immediate challenges, which may include serving as the Clinician of the Day, offering milieu support, and being readily available for urgent interventions.

• Documenting and monitoring client progress through electronic health records, ensuring compliance with all governance and ethical standards while maintaining accurate patient histories.

• Collaborating with other mental health professionals and community resources to develop and implement personalized care plans tailored to the unique needs of patients.

• Engaging in ongoing training and professional development to enhance your clinical skills and knowledge of best practices in telehealth delivery.
