Key Responsibilities\n

• Conduct comprehensive patient intakes, perform diagnostic and functional evaluations, and collaborate with primary care providers to develop tailored treatment plans.

• Provide brief, focused interventions to patients in need of mental health support, using therapeutic techniques grounded in current best practices.

• Monitor ongoing patient progress, initiating follow-ups to assess the effectiveness of treatment approaches and making adjustments as necessary.

• Develop and implement relapse prevention plans to help patients maintain their mental health and prevent deterioration of their conditions.

• Identify “at risk” patients and work alongside the care team to formulate preventative strategies and treatment modalities.

• Coordinate and document all health services related to behavioral health care, ensuring seamless communication with other treatment providers.

• Educate patients on self-management skills to empower them in their treatment journey, allowing them to take an active role in their recovery.

• Consult with clinic management and team members regarding behavioral health services and contribute to program evaluation and improvement initiatives.

• Support patients in adhering to medical treatments prescribed by their primary care provider.

• Ensure compliance with the Code of Ethical Conduct for Clinical Professionals and maintain accurate clinical records.

The LPC role at Crossbend Behavioral Center offers a competitive salary ranging from USD 87,000 to 105,000 annually, depending on experience and qualifications.
