Key Responsibilities\n

• Conduct comprehensive assessments for clients to thoroughly understand their social, emotional, and psychological needs, ensuring tailored support and intervention.

• Provide individual and group counseling sessions, implementing evidence-based therapeutic approaches such as Cognitive Behavioral Therapy (CBT) and Solution-Focused Therapy.

• Develop and implement personalized service plans, collaborating with clients to set realistic goals and objectives, driving meaningful progress.

• Advocate for clients’ rights and access to necessary resources, addressing barriers that may impede their progress toward healing and empowerment.

• Contribute actively to program development and evaluation, playing a key role in grant writing and reporting initiatives that enable the organization to secure funding.

• Engage with community partners and volunteers to enhance service delivery, broadening the impact of our collective efforts in the community.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals and families facing a range of challenges associated with grief, emotional distress, chronic illness, and end-of-life issues within a hospice care context. As the population can be diverse, you must adapt your therapeutic approaches to accommodate clients' varying cultural backgrounds, life experiences, and emotional needs, ensuring a holistic and empathetic response to each unique situation.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current and active state licensing as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
