Key Responsibilities\n

• Conduct comprehensive psychosocial assessments by utilizing interviews, observations, and reviewing medical records to understand the emotional, social, and environmental factors impacting patients’ recovery.

• Provide empathetic counseling and support services to patients and their families, particularly concerning adjustment issues related to their health and emotional well-being.

• Collaborate closely with the rehabilitation team to develop individualized treatment plans that prioritize the psychosocial aspects of recovery.

• Facilitate access to community resources, including support groups, vocational rehabilitation services, and housing assistance, helping patients navigate their post-rehabilitation journeys.

• Educate and empower patients and families regarding community resources and the range of post-rehabilitation services available to them.

• Actively participate in discharge planning, ensuring smooth transitions for patients back to home or alternative care settings.

Patient Population / Client Focus\n

Our patient population encompasses a wide range of individuals recovering from various conditions, including strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

Compensation and Benefits\n
