Key Responsibilities\n

• Conduct comprehensive assessments and develop tailored treatment plans to effectively meet the unique needs of each patient.

• Maintain ongoing communication with supervisors, direct care staff, family members, and guardians regarding the progression of individual treatment plans and any pertinent changes in circumstances.

• Organize and provide a detailed schedule of client appointments, home visits, and clinical observations to ensure accountability and facilitate optimal patient care.
\n
• Coordinate home visits as necessary, involving chart reviews, staff interviews, and thorough data analysis to gain insights into the client’s environment and well-being.
\n
• Ensure the successful implementation of established program plans, monitoring their effectiveness through regular evaluations.
\n
• Participate in Interdisciplinary Team Meetings and other collaborative discussions to align treatment goals and share insights.
\n
• Prepare and submit detailed monthly and quarterly progress notes to maintain accurate patient records.
\n
• Lead training sessions to certify staff members in the execution of program plans and effective documentation practices.
\n
• Conduct follow-up interviews and home visits, ensuring ongoing support aligns with therapeutic goals.
\n
• Act as a liaison between the mental healthcare community and the public to enhance support services and development initiatives.

• Collaborate in the development of procedures for training materials aimed at improving programs and client care.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
