Key Responsibilities\n

• Psychosocial Assessment: Conduct timely psychosocial assessments by leveraging your theoretical knowledge of human behavior and psychosocial influences on treatment outcomes. Remain vigilant regarding ethical considerations and potential risks that may impact your assessments.

• Clinical Intervention: Provide supportive counseling to patients and their families, facilitate family conferences, and assist in linking them with relevant community agencies. Help families navigate complex decisions, such as hospice care or DNR orders, while ensuring effective communication with the healthcare team.

• Triage and Management: Skillfully manage your clinical caseload by identifying high-risk situations and effectively balancing referrals alongside ongoing therapy sessions. Engage with colleagues and supervisors to optimize patient care.

• Documentation: Maintain accurate records by documenting all interactions and interventions in compliance with organizational policies, including Interdisciplinary Plans of Care and Social Services Progress Notes.

• In-Service Training: Facilitate training sessions for hospital staff on key psychosocial issues impacting patient care, enhancing the overall understanding of social work within the clinical setting.

• Quality Improvement: Participate actively in quality improvement initiatives by analyzing process outcomes that significantly enhance patient care and satisfaction.

• Competency Maintenance: Stay updated on clinical practices and maintain competencies aligned with hospital standards, focusing on age-specific and job-specific care protocols.

• Additional Duties: Engage in community programs, support groups, and events to foster a supportive environment for patients and families.

Qualifications\n

Compensation and Benefits\n

The annual base salary for this position ranges from USD 120,000 to 130,000 , commensurate with experience and relevant qualifications.
