Key Responsibilities\n

• Conduct ongoing assessments to identify and address the evolving needs of patients, ensuring comprehensive care tailored to individual requirements.

• Facilitate clear communication with supervisors, direct care staff, family members, and guardians, providing updates on patient progress to create a collaborative care environment.

• Develop and propose tentative schedules for appointments, home visits, and observations, supporting the overall monitoring of patient advancement.

• Oversee and coordinate home visits, ensuring adherence to in-home observations, chart reviews, and staff interviews, resulting in accurate data analysis for informed decision-making.

• Ensure that program plans are effectively implemented and regularly evaluate their impact on patient outcomes.

• Actively participate in interdisciplinary team meetings, contributing insights and recommendations based on clinical experience and ongoing assessments.

• Document monthly and quarterly progress through detailed notes, providing accountability and continuity of care for each individual.

• Organize and lead training sessions to certify that staff and other team members are fully competent in implementing program plans and maintaining documentation standards.

• Provide ongoing support through follow-up interviews, home visits, and observations, remaining responsive to the individual needs of patients.

• Maintain meticulous records to ensure information is accessible to relevant parties involved in patient care.

• Serve as a liaison to the public and mental health community, fostering collaborations and supporting development goals that benefit our clients.
