Work Environment

This role is primarily onsite, with the unique setting of home care allowing therapists to deliver individualized care in the comfort of patients' homes. Our team operates from Monday to Friday, 9 AM – 5 PM, providing a structured yet supportive work environment. Key Responsibilities

• Conduct comprehensive evaluations to assess each patient's functional abilities, identifying specific rehabilitation needs and goals.
• Develop and implement customized treatment plans that incorporate evidence-based interventions tailored to each patient's condition.
• Utilize a variety of manual therapy techniques, therapeutic exercises, and wellness strategies to improve mobility, alleviate pain, and enhance quality of life.
• Work closely with a collaborative team of healthcare professionals, including physicians and occupational therapists, to ensure comprehensive care for every patient.
• Educate patients and their families on effective therapeutic exercises, strategies for home care, and injury prevention techniques, fostering self-management of their conditions.
• Maintain thorough and accurate documentation of patient assessments, treatment plans, and progress reports in compliance with regulatory standards, ensuring continuity of care.

Patient Population / Client Focus

In this role, you will primarily engage with diverse patient populations, including individuals recovering from orthopedic surgeries, injuries, or chronic conditions affecting their mobility and quality of life. Your expertise will help guide these patients through their rehabilitation journeys, empowering them to reach their personal health objectives.

Qualifications

• Master of Physical Therapy (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Active Physical Therapy License in the state of New Jersey.
• Minimum of 2 years of clinical experience, ideally in outpatient or orthopedic settings.
• Current Basic Life Support (BLS) or CPR certification for healthcare professionals.
• Strong communication and interpersonal skills to build rapport with patients and collaborate effectively within a multidisciplinary team.

Preferred Experience

• Experience working in home care settings is highly desirable, providing insights into patient needs and effective therapy adaptations in a residential environment.
• Knowledge of orthopedic rehabilitation protocols and familiarity with evidence-based practices in physical therapy.

Compensation and Benefits
