Work Environment

As a part of our Home Care division, you will operate primarily in the homes of clients located in Freehold and nearby areas. This setting allows you to provide personalized care tailored to the individual needs of each patient. The role involves working closely with an interdisciplinary team, healthcare providers, and family members, all while managing flexible hours that accommodate patient needs, including variable weekends.

Key Responsibilities

Your daily responsibilities will include:

• Conducting initial patient assessments to evaluate physical abilities, limitations, and potential recovery trajectories.
• Developing comprehensive treatment plans that address specific functional limitations and disabilities.
• Implementing therapeutic interventions based on best practices, regularly re-evaluating patients to ensure treatment efficacy.
• Documenting evaluations, treatment sessions, and discharge plans in compliance with all healthcare regulations and licensing requirements.
• Providing guidance and consulting with healthcare providers on patient care strategies and continuum of care.
• Supervising and mentoring support staff, enhancing their effectiveness and promoting a cohesive care environment.
• Engaging in ongoing education and self-improvement activities to maintain licensure and improve clinical skills.

Patient Population / Client Focus

You will be working with a diverse patient population consisting of individuals with orthopedic injuries, neurological conditions, and post-operative rehabilitation needs. As a physical therapist, your role is pivotal in assisting these patients to regain independence and improve their quality of life through tailored therapy interventions.

Qualifications

The successful candidate will possess the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid physical therapy state license in New Jersey.
• Basic Life Support (BLS) certification.
• Strong written, verbal, and interpersonal communication skills to effectively interact with patients and interdisciplinary team members.
• Proficiency in using electronic health record systems and basic computer skills for documentation purposes.

Preferred Experience

We prefer candidates with at least two years of experience in a physical therapy role, particularly in a home care or outpatient setting, as this experience will enhance your ability to deliver effective care in patient homes. Experience with specialized rehabilitation techniques and care coordination is a plus.

Compensation and Benefits
