Work Environment

This position is primarily onsite and involves working in patients' homes. You will encounter various environments within the home care setting, tailoring your methodologies to facilitate recovery in comfortable and familiar surroundings for each patient. The typical work schedule is Monday through Friday, from 9:00 AM to 5:00 PM, enabling a structured yet rewarding work-life balance.

Key Responsibilities

• Conduct thorough assessments of patients' physical capabilities and establish their rehabilitation goals, ensuring that treatment is aligned with their individual needs.
• Develop innovative care plans utilizing evidence-based practice and a range of therapeutic modalities to enhance patients' movement capabilities.
• Implement treatment strategies that improve motion and mobility, while fostering patients' independence and confidence during their rehabilitation journey.
• Monitor patient progress closely, making necessary adjustments to treatment plans and keeping patients informed about their advancements.
• Provide ongoing education to patients regarding exercises, wellness strategies, and lifestyle changes to promote long-term health and fitness.
• Motivate and encourage patients throughout the treatment process to help them achieve optimal functionality.
• Collaborate effectively with a multidisciplinary team of healthcare professionals to ensure comprehensive holistic care.

Patient Population / Client Focus

Your role will primarily focus on patients with orthopedic conditions, including musculoskeletal injuries, post-operative rehabilitation, and chronic pain management. As a Physical Therapist in home care, you will frequently encounter diverse patient demographics, ranging from athletes recovering from injuries to older adults aiming to regain mobility. Your compassionate approach will be critical in addressing the unique needs of each client and fostering a supportive rehabilitation environment.

Qualifications

• Must possess a Master's Degree or Doctorate in Physical Therapy from an accredited program.
• Active state licensure as a Physical Therapist.
• A strong foundation in orthopedic rehabilitation, with at least 2 years of experience treating musculoskeletal issues.
• Proficient in evaluating patient needs and developing targeted therapy plans to address their specific challenges.
• Exceptional communication skills, capable of effectively engaging patients, families, and other healthcare providers.
• Adept at managing diverse personalities and patient scenarios, demonstrating patience and adaptability.

Preferred Experience

• Experience working in a home care setting is highly desirable, with a focus on creating patient-centered treatment plans.
• Familiarity with various physical therapy modalities, including manual therapy techniques, therapeutic exercise, and wellness programming.

Compensation and Benefits

We offer a competitive annual salary ranging from $110,000 to $130,000, complemented by a robust benefits package.
