Key Responsibilities\n

• Conduct thorough psychosocial assessments through structured interviews, detailed observations, and in-depth reviews of medical records, to evaluate emotional, social, and environmental factors impacting patients’ recovery journeys.

• Provide compassionate counseling and psychological support to patients and their families, addressing significant adjustment issues, emotional challenges, and fostering resilience.

• Collaborate closely with the rehabilitation team to create personalized treatment plans that consider each patient’s psychosocial needs, ensuring the incorporation of therapeutic goals into their overall rehabilitation strategy.

• Assist patients in navigating community resources, such as support groups, vocational rehabilitation services, and housing assistance, to promote their holistic recovery.

• Educate patients and their families about available community resources and post-rehabilitation services that facilitate long-term wellness and independence.

• Participate actively in discharge planning processes, coordinating proper transitions to home or alternative care settings to ensure continuity of care.

Patient Population / Client Focus\n

You will work with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

Compensation and Benefits\n
