Work Environment\n

This role offers a remote working arrangement, which enhances flexibility while providing valuable support to clients. You will primarily work from home but will be expected to maintain high communication standards with interdisciplinary teams through virtual meetings and consultations. You’ll play a pivotal role in coordinating care among healthcare professionals, caregivers, and clients’ families, ensuring that every client receives personalized attention and care. Our work hours are typically Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend flexibility based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the psychosocial needs of seniors, incorporating emotional, mental, and social factors.

• Develop and implement tailored care plans in collaboration with clients and their families, ensuring that individual goals and preferences are prioritized.

• Provide ongoing counseling and emotional support, helping seniors navigate transitions such as loss, relocation, or health changes.

• Advocate for clients’ rights and facilitate their access to essential community resources and services.
\n
• Regularly monitor client progress through home visits to assess living conditions, address concerns, and provide necessary interventions.

• Facilitate effective communication among family members and resolve conflicts related to the care of senior clients.

• Educate clients and families about available community resources, support groups, and programs to promote social engagement.

• Maintain detailed and accurate documentation of assessments, care plans, and client interactions within electronic health record (EHR) systems.

• Stay updated with the latest social work practices and regulations to ensure compliance with ethical standards.

Your primary focus will be on seniors who may face various life challenges such as health-related issues, mental health concerns, and difficulties associated with aging. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The salary for this position is competitive, ranging from $60,000 to $100,000 annually, based on experience and qualifications.
