Key Responsibilities\n

• Conduct thorough assessments of clients’ mental and emotional needs, utilizing evidence-based assessment tools to inform treatment planning.

• Provide individual and group therapy sessions aimed at helping clients navigate grief, depression, anxiety, and other psychological difficulties related to end-of-life issues.

• Maintain accurate, up-to-date clinical documentation, including progress notes and treatment protocols, in compliance with organizational and regulatory standards.

• Communicate effectively with medical providers to stay updated on clients’ conditions and adjust therapeutic interventions as needed.

• Respond promptly to clients experiencing crises, offering immediate support and developing safety plans to mitigate risks.

• Facilitate communication with families to involve them in treatment planning and support their understanding of clients’ needs and progress.

Patient Population / Client Focus\n

You will primarily work with patients in a hospice setting, focusing on individuals dealing with terminal illnesses and their accompanying emotional challenges. This role requires sensitivity to the unique needs of clients navigating this difficult phase in their lives, helping them come to terms with their conditions, manage pain, and maintain dignity as they transition.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related field is required.

• Active state licensure as an Independent Clinical Social Worker (LICSW) or equivalent is mandatory.

• At least three years of experience in an outpatient mental health setting is essential.

• Strong skills in working independently with effective self-management and organizational abilities.

• Proficiency in utilizing computer-based documentation systems, electronic medical records, and standard office software applications, including email and spreadsheets.

Compensation and Benefits\n
