Full-Time Physical Therapist - Onsite at Skilled Nursing Facility

Position Overview

We are looking for a dedicated and licensed Physical Therapist to join our team at Work It PT, a prestigious Skilled Nursing Facility in Montgomery, New York. The primary focus of this role is to deliver high-quality rehabilitative care tailored to meet the individual needs of our residents. Work Environment

This is an onsite role, where you will work directly with residents in our well-equipped Skilled Nursing Facility. The work schedule is full-time, with hours from Monday to Friday, 9 AM to 5 PM. Collaborating closely with other healthcare professionals, including nurses, occupational therapists, and rehabilitation aides, you will create a holistic approach to patient care that emphasizes teamwork and interdisciplinary collaboration.

Key Responsibilities

• Conduct comprehensive evaluations to assess residents' physical conditions and functional limitations, which will inform your treatment decisions.
• Design, implement, and adjust individualized therapy plans that facilitate recovery and improve residents' mobility and quality of life.
• Administer therapeutic exercises, manual therapy techniques, and electrical modalities as appropriate to manage pain and enhance rehabilitation outcomes.
• Document patient progress meticulously and adjust treatment plans based on data collected during therapy sessions.
• Work collaboratively in an interdisciplinary team setting, coordinating care with physicians, nurses, and other therapists to ensure optimal patient outcomes.
• Educate patients and their families on home exercise programs and effective strategies for enhancing recovery outside of the facility.
• Stay compliant with all facility protocols, state regulations, and best practice guidelines to ensure the safety and effectiveness of care provided.

Patient Population / Client Focus

You will primarily work with elderly residents and individuals recovering from surgery or serious health conditions that have impacted their mobility. Your role will involve understanding the specific challenges faced by these patients, including post-operative recovery, chronic pain management, and rehabilitation from strokes or other debilitating illnesses. Your compassionate and thoughtful approach will be essential for building rapport and trust with residents and their families.

Qualifications

• A Bachelor's Degree in Physical Therapy or higher-level degree (Master's or Doctorate) from an accredited physical therapy program.
• An active and unrestricted Physical Therapy license in the state of New York.
• Two years of experience as a Physical Therapist, preferably within a skilled nursing facility or rehabilitation setting.
• Exceptional clinical skills paired with strong communication and interpersonal abilities.
• A commitment to a patient-centered approach that emphasizes empathy and respect.

Preferred Experience

While a minimum of two years of relevant experience is required, candidates with experience in skilled nursing or geriatric therapy settings will be given preference. Familiarity with electronic health records (EHR) and rehabilitation software is also advantageous, as it will aid in the documentation and tracking of patient progress.
