Key Responsibilities

• Assess patient conditions and devise personalized treatment plans that address their unique needs and goals.
• Provide hands-on therapy, including manual therapy techniques, therapeutic exercises, and mobility training to a caseload of up to 7 patients per shift.
• Maintain clear and compassionate communication with patients and their families, offering educational resources regarding treatment protocols and progress updates.
• Document patient progress and treatment outcomes accurately and in a timely manner, using our electronic health record (EHR) system.
• Collaborate with interdisciplinary team members to refine therapeutic approaches and discuss patient care strategies.
• Participate in staff training and development programs to enhance skills and stay updated on industry best practices.

Patient Population / Client Focus

As a Physical Therapist at Moving & Grooving Rehab Center, you will work with a diverse population of patients, including those recovering from surgery, traumatic injuries, and chronic pain conditions. Your expertise will play a vital role in assisting elderly patients within our Skilled Nursing Facility to achieve their rehabilitation goals, focusing on enhancing their quality of life and independence.

Qualifications

• Active Physical Therapist License in the state of New York.
• A Bachelor's degree in Physical Therapy is required; a Master's or Doctorate in Physical Therapy is preferred.
• At least 2 years of experience in a clinical or rehabilitation setting, demonstrating proficiency in both hands-on therapy and patient assessment.

Preferred Experience

• Experience working in a skilled nursing facility or rehabilitation context is highly desirable.
• Familiarity with electronic health record systems is beneficial for seamless documentation.
• Experience with fall prevention strategies and knowledge of geriatric care principles will be an asset.

Compensation and Benefits
