Full-Time Nurse Practitioner at Atlantis Athletes Physical Therapy Care

Position Overview

We are seeking a dedicated and compassionate Nurse Practitioner to join our dynamic team at Atlantis Athletes Physical Therapy Care in Olean, New York. This full-time position offers the opportunity to provide exceptional, value-based primary care to our adult patients, particularly those on Medicare. You will work collaboratively within an interprofessional team to enhance patient care, focusing on prevention and health management to minimize hospital admissions.

Work Environment

This role is based onsite at our outpatient office, where you will work Monday through Friday from 9:00 AM to 5:00 PM. The office environment is collaborative, staffed with a team that includes physicians, physician assistants, medical assistants, and health care managers. Our facility is equipped with the latest medical tools and technologies to support your practice and enhance patient interactions.

Key Responsibilities

As a Nurse Practitioner at Atlantis Athletes Physical Therapy Care, your core responsibilities will include:

• Conducting thorough office visits for both routine and acute patient issues, ensuring that every patient's concerns are addressed comprehensively.
• Administering annual wellness visits and health risk assessments, which involve a holistic and patient-centered approach to health management.
• Leading care coordination efforts with other healthcare providers, specialists, and community resources to ensure seamless patient experiences and continuity of care.
• Taking charge of population health initiatives, such as ensuring appropriate screenings are performed in line with clinical guidelines.
• Engaging in proactive phone triage and outreach to support our patients’ needs and educate them on their health conditions.
• Participating in community outreach and promotional activities to raise awareness about our services and engage with the local community.
• When necessary, conducting home visits to patients who may have mobility challenges or require additional support in their care.
• Reporting to the Medical Director and closely collaborating with operational leadership to improve clinical practices and patient outcomes.

Patient Population / Client Focus

Your primary focus will be on providing quality healthcare to older adults on Medicare. This demographic often faces unique health challenges, and your role will involve fostering trust and building meaningful relationships to empower these patients in managing their health proactively. Your expertise will contribute significantly to helping them maintain their health and well-being, allowing them to live fulfilling lives.

Qualifications

To qualify for this role, candidates must possess:

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active and non-probationary state Nurse Practitioner license, along with an active DEA license.
• A proven background in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Strong knowledge of HCC documentation, ICD-10 coding practices, and Health Risk Assessments.
• A passion for collaboration and teamwork in a multidisciplinary environment.

Preferred Experience

Experience working with older adult populations, coupled with familiarity in utilizing electronic health records (EHR) for patient management and documentation, will be highly advantageous. Previous exposure to quality improvement initiatives in healthcare delivery will set candidates apart.

Compensation and Benefits

The salary range for this position is USD 120,000 – 160,000 annually, commensurate with experience.
