Brightwood Orthopedic and Family Medicine Care is seeking a dedicated and skilled Nurse Practitioner (NP) to join our dynamic team in Olean, New York. As a full-time NP specializing in Family Medicine, you will play a crucial role in providing high-quality, patient-centered care to adults, particularly those on Medicare. This position emphasizes both independent practice and collaborative care, accommodating various patient needs while fostering long-term patient relationships.

Work Environment

You will work in a supportive outpatient office environment, collaborating with a multidisciplinary team that includes physicians, other Nurse Practitioners, Physician Assistants, medical assistants, and registered nurses. Our clinic is equipped with dedicated medical scribes, enabling you to focus more on patient care while ensuring all documentation and coding requirements are met effectively.

Key Responsibilities

• Deliver exceptional primary care services, addressing both routine and acute health issues.
• Conduct comprehensive annual wellness visits and health risk assessments, ensuring thorough documentation and a holistic view of patient health.
• Coordinate care with other healthcare providers and specialty services to optimize patient outcomes.
• Lead population health initiatives, ensuring adherence to evidence-based screening guidelines, such as breast cancer screening for eligible women.
• Assist in phone triage and outreach to enhance patient engagement and health education.
• Educate patients on their health conditions, care plans, and treatment options, fostering trust and understanding.
• Participate in promotional activities that enhance community awareness of our healthcare services.
• Conduct home visits as necessary to provide care and support for patients with mobility challenges.
• Report to the Medical Director and collaborate closely with operational leadership to drive clinical excellence.

Patient Population / Client Focus

You will primarily serve older adults on Medicare, committing to keeping them healthy and engaged in their care. Our focus on personalized primary care enables us to address the multifaceted needs of our patients, ensuring they have the knowledge and support necessary for a fulfilling life.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or equivalent qualification from an accredited NP program.
• Active national certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Current, non-probationary state Nurse Practitioner license and active DEA license.
• A genuine passion for providing high-quality primary care to diverse populations.
• Strong capability in understanding HCC (Hierarchical Condition Categories) documentation, ICD-10 Coding, and conducting Health Risk Assessments (HRAs).

Preferred Experience

• Experience in primary care settings, including specialties such as internal medicine, geriatrics, or family medicine.
• Previous involvement with innovative care models focused on population health.
• Bilingual proficiency in relevant languages that cater to our diverse patient demographic.

Compensation and Benefits

The annual salary for this role ranges from USD 115,000 to 150,000, depending on experience and expertise. In addition, we offer a comprehensive benefits package that includes:
