Position Overview

State Health, LLC is seeking a dedicated and compassionate Nurse Practitioner specializing in Internal Medicine to join our growing team in Olean, New York. This full-time position will allow you to utilize your expertise in providing high-quality primary care to our Medicare patients, ensuring they remain healthy, happy, and out of the hospital.

Key Responsibilities

As a Nurse Practitioner at State Health, you will:

• Provide exceptional primary care by conducting thorough office visits for both routine and acute health concerns.
• Administer annual wellness visits and health risk assessments, with an emphasis on holistic patient evaluations and meticulous documentation.
• Coordinate patient care with other providers, specialists, testing facilities, and community agencies to ensure seamless healthcare delivery.
• Lead population health initiatives, ensuring compliance with evidence-based practices for preventive screenings and assessments.
• Assist the care team with phone triage and patient outreach efforts to enhance care continuity.
• Educate patients about their health conditions, care plans, and treatment options, empowering them to take an active role in their health.
• Engage in promotional activities aimed at community health enhancement.
• Conduct home visits when required, ensuring that our care reaches patients where they feel most comfortable.
• Report directly to the Medical Director and maintain close collaboration with operational leadership.

Patient Population / Client Focus

Our primary focus is on Medicare patients, many of whom are older adults living with complex health conditions. Qualifications

Qualified applicants for the Nurse Practitioner role should possess:

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner by either AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• An active DEA license to prescribe medications.

Preferred Experience

Preference will be given to candidates with:

• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and Health Risk Assessments (HRAs).
• Ability to work effectively in a collaborative, cross-functional team environment driven by a passion for innovative healthcare delivery.
• Bilingual proficiency in relevant languages that enhance communication with diverse patient populations.

Compensation and Benefits

Our competitive salary range for this position is between $115,000 to $150,000 annually, with the potential for wider compensation based on relevant experience and expertise. In addition to a robust salary, we offer a comprehensive benefits package, which includes:
