Work Environment\n

As a remote position, this role offers the flexibility to work from home while maintaining an engaging and productive communication structure with clients and coworkers. The position requires availability during standard business hours, specifically Monday to Friday from 8:00 AM to 4:30 PM, with occasional flexibility for evening or weekend outreach based on client needs. You may also be required to be on-call to provide timely support during urgent situations. Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors that affect their well-being.

• Collaborate closely with clients and their families to craft personalized care strategies and action plans that align with their goals and preferences.

• Provide emotional support and guidance to seniors, helping them navigate transitions, challenges, and changes in their personal lives.

• Act as an advocate for your clients, ensuring their rights are upheld and that they have access to vital community resources and services.

• Facilitate communication between family members and care teams to resolve conflicts and promote collective understanding in senior care situations.

• Document client assessments, care plans, and interaction notes accurately in our electronic health record (EHR) systems, maintaining up-to-date information for consistent care coordination.

• Monitor the progress of clients through regular follow-ups, identifying needs and making necessary adjustments to care plans based on their evolving situations.

• Educate clients and their families on available community resources, support groups, and programs that promote social engagement and a higher quality of life.

• Stay informed about current best practices and regulatory standards in social work, ensuring compliance and ethical conduct at all times.

Patient Population / Client Focus\n

In this role, you will primarily focus on seniors who experience a variety of psychosocial and emotional challenges. This includes issues related to aging, mental health disorders, significant life transitions, chronic illnesses, and the challenges of maintaining independence. Your expertise will help in assessing their needs holistically while ensuring they have access to support systems that facilitate their comfort and happiness.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Must hold a current and valid state licensure as a Licensed Master Social Worker (LMSW).

• At least two years of relevant experience in social work, particularly in settings that emphasize geriatric care or community services.

• Proficient knowledge of social work principles, issues specific to the aging population, and available local community resources.

Compensation and Benefits\n

This position offers competitive annual compensation ranging from $60,000 to $100,000, commensurate with experience and qualifications.
