Key Responsibilities\n

• Educate patients and their families about available community resources and post-rehabilitation services to empower them in maintaining their health and well-being after discharge.

• Participate actively in discharge planning processes to ensure a smooth transition to home or other care settings, addressing potential challenges and planning for ongoing support.
