Work Environment\n

This role offers a remote work setting, allowing for flexibility in your schedule while ensuring that you maintain a close connection with clients through regular check-ins and support. While the majority of work can be performed from home, there may be occasional visits to clients' homes or care facilities to assess their needs firsthand. Our standard operational hours are Monday to Friday from 8:00 AM to 4:30 PM, and occasional evenings or weekends may be required based on client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, utilizing standardized assessment tools to evaluate emotional, social, and mental health factors.

• Develop individualized treatment plans in collaboration with clients and their families, ensuring alignment with their goals, preferences, and care needs.

• Provide counseling services that help clients navigate transitions related to aging, health challenges, and end-of-life concerns.

• Act as an advocate for clients, ensuring they have access to necessary community resources, services, and support systems.

• Regularly monitor client wellbeing through home visits and virtual check-ins, addressing any concerns or safety issues they may face.

• Document all client interactions comprehensively using our electronic health record (EHR) system, ensuring accuracy and compliance with regulatory standards.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.
