Work Environment\n

The LCSW role is structured around a fully remote work environment, with interactions taking place through telehealth platforms and in-home visits as needed. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children enrolled in hospice programs.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring that each plan aligns with both the child's needs and family priorities.

• Monitor and evaluate outcomes in collaboration with an interdisciplinary team, making necessary adjustments to support the child's development effectively.

• Provide emotional and educational support to families, equipping them with strategies and resources to enhance their child's growth.

• Utilize evidence-based practices and adhere to Natural Learning Environment Practices to create engaging and supportive learning experiences.

• Build rapport and maintain open communication with children, families, and team members to foster a healthy and productive relationship.

• Ensure compliance with all federal and state regulations regarding social work practices.

Patient Population / Client Focus\n

This position primarily focuses on children within the hospice care system and their families. You will work closely with infants and toddlers experiencing disabilities or developmental delays, helping them acquire essential skills in a nurturing and supportive environment. The work involves addressing the emotional, cognitive, and social needs of children during a critical time in their lives.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• A valid driver's license as travel may be required for home visits.

• One year of experience working with young children, particularly in early intervention settings.
