Key Responsibilities\n

• Collaborate with patients and the healthcare team to develop personalized care plans aimed at achieving rehabilitation and treatment goals.

• Utilize evidence-based patient education programs and social work methodologies to identify barriers and facilitate effective treatment interventions.

• Provide psychosocial support and monitoring to patients adjusting to new health challenges, ensuring they achieve optimal quality of life.

• Deliver compassionate counseling services, including goal-directed support for patients seeking transplant options.
\n
• Assist patients in accessing community resources such as home health services and vocational rehabilitation, promoting better treatment outcomes.

• Maintain up-to-date knowledge of local rehabilitation programs to connect patients with employment and educational opportunities.
\n
• Support patients and families in understanding advance directives, facilitating necessary discussions to clarify wishes and ensure compliance with legal guidelines.

• Assess and document patients’ psychosocial strengths and needs comprehensively, contributing to the overall patient assessment conducted by the interdisciplinary team.

Patient Population / Client Focus\n

The primary focus of our Mental Health Counselors is to support a diverse patient population struggling with a variety of mental health issues, including anxiety, depression, and adjustment disorders, particularly in the face of health challenges such as chronic illness and kidney disease. You will engage with patients from all walks of life, valuing their unique experiences and working diligently to meet their individual needs.

Qualifications\n

• A Master’s degree in Psychology, Counseling, Social Work, or a related field.

• A state-issued social work license, such as LMHC, LMFT, or LICSW, is required.

• A clear record free from medical malpractice claims verified by a third party.

• Authorization to work in the U.S.

Compensation and Benefits\n
